Insurance Customer Privacy Notice

This notice describes Northdale insurance information practices, including federal privacy protections and additional California rights. It is intended to accompany insurance transactions and to be available on the website.
About this notice
Northdale Insurance Agency, Inc. collects and uses information to help people obtain and service insurance. This notice explains the categories of nonpublic personal information we handle, our disclosure practices, and the choices and rights available under applicable insurance privacy law. Nonpublic personal information generally means personally identifiable information that is not lawfully public, including information obtained in an insurance transaction.
Federal financial privacy protections generally address individuals obtaining products or services primarily for personal, family or household purposes. Commercial insurance, business entities and information about drivers or other individuals can be treated differently under state law. We protect commercial account information under our Privacy Policy and honor insurance information rights when they apply. We do not treat every person connected to a commercial policy as outside all privacy protections.
This notice applies to Northdale. It does not replace an insurer's, premium finance company's or another provider's own privacy notice. California rights described here are not limited by a separate federal or standard privacy notice that we or another institution provide.
Information we collect and the sources we use
The information needed depends on the insurance product and your transaction. We may collect contact and identification details; household, beneficiary and applicant information; vehicle, driver, property and business information; prior insurance and claims history; application responses; premium, billing and finance records; and communications and instructions. Health or medical information may be needed for certain life or other insurance applications and is handled through an appropriate authorization and collection process.
We obtain information directly from you or an authorized representative and, where permitted, from insurance institutions, other agents, insurance support organizations, consumer reporting agencies, motor vehicle records, inspection providers, employers or business representatives, public records and medical professionals. Information may be collected from someone other than the person proposed for coverage. We may use applications, questionnaires, record checks, inspections and verification inquiries appropriate to the transaction. Any investigative consumer report or medical disclosure requiring a separate notice or authorization is handled separately.
An insurance support organization that prepares a report may retain the information it obtains and disclose that information to other persons as permitted by law. A consumer reporting agency's records and dispute procedures may be subject to separate requirements.
Why information is used
We use information to seek and compare insurance options; prepare or transmit applications; assist with underwriting questions, issuance and renewal; service policy changes and certificates; support claim reporting; process authorized billing or finance arrangements; prevent fraud; and meet recordkeeping and legal obligations. The carrier determines underwriting and claim coverage under the policy and applicable law. Privacy choices do not authorize us to alter a carrier's records or determinations beyond our authority.
Our sharing practices
The following table describes our practices. Our disclosures remain subject to any stricter state restriction, authorization requirement or opt-out right.

An affiliate is a company related by common ownership or control. A nonaffiliate is a company that is not related by common ownership or control. A service provider performing work for us is not thereby authorized to market independently. An insurer to which we submit your application can have its own legal role and privacy obligations.
We do not currently share your information with affiliates. If our disclosure practices change in a way that requires a revised notice, consent or a meaningful opt-out opportunity, we will provide that before beginning the affected disclosure. We do not sell, rent or disclose SMS opt-in data to third parties or affiliates for their own marketing.
Disclosures permitted for insurance business
Subject to applicable law, routine disclosures may be made without a separate authorization to persons performing professional, business or insurance functions on our behalf; to insurance institutions, agents and insurance support organizations for an insurance transaction or fraud prevention; to insurance regulators or other authorities; to comply with lawful process; and in connection with a permitted transfer of an insurance business. We disclose only information appropriate for the permitted purpose and apply any restrictions on the recipient's further use.
Where a state requires authorization for a disclosure, we obtain it before disclosing. This notice is not itself your consent to release medical records, obtain every type of consumer report, or share information for unrelated marketing. We do not use the existence of a website privacy policy to bypass insurance-specific restrictions.
Confidentiality and former customers
We use reasonable administrative, technical and physical safeguards. Access is limited according to job and service responsibilities. Providers handling information for us are expected to use appropriate confidentiality and security measures. We continue to apply this notice to retained information about former customers. Ending a policy does not automatically require deletion of records that must be retained.
California access rights
If California Insurance Code section 791.08 applies, you may submit a written request reasonably describing recorded personal information about you that we can reasonably locate and retrieve. After proper identification, we will respond within 30 business days after receiving the request as the law requires. You may inspect and copy covered information or obtain a copy by mail, subject to statutory limitations.
The response includes required information about institutional sources and, if recorded, recipients to whom covered information was disclosed during the preceding two years. If recipient identities were not recorded, the law provides for identification of the usual recipient categories. We will also explain the process for requesting correction, amendment or deletion. Any fee will be limited to what law permits, and we will explain it before charging it.
Special rules may apply to medical records, mental health records, privileged information and information collected in connection with or in anticipation of a claim or proceeding. We will explain an applicable restriction rather than describe a blanket right to every document in an insurance file.
California correction and disagreement rights
You may make a written request to correct, amend or delete covered recorded personal information you believe is inaccurate, irrelevant or unfair. Where section 791.09 applies, within 30 business days after receiving your request we will make the change or explain our refusal, its reasons and your right to submit a statement of disagreement.
If you disagree with a refusal, you may provide a concise statement of the information you believe is accurate, relevant or fair and the reasons for your disagreement. We will associate the statement with the disputed information and include it in later disclosures and notifications as required by law. If we correct, amend or delete information, we will provide required notices to specified previous recipients and insurance support organizations. These rights are subject to the statutory scope and exceptions and do not create a general right to erase an accurate insurance history.
Additional state rights and marketing choices
Residents of other states may have additional insurance privacy rights, restrictions on disclosure, access or correction procedures, or authorization and opt-out requirements. We follow the law applicable to the person, information and transaction. If a state requires more protection than this notice describes, that protection applies. You may ask us to stop optional Northdale marketing at any time. We do not condition insurance services on consent to disclosure for unrelated marketing where prohibited. A marketing opt-out does not prevent disclosures needed to obtain or service insurance, comply with law or carry out an authorized transaction. Requests and complaints

State your name, contact information, the policy or transaction if known, and the information or correction requested. Do not send a Social Security number or identification copy unless we request it through an appropriate verification method. Call (559) 777-7657 for assistance preparing or directing a request.
We will verify identity and any representative's authority in a manner proportionate to the request. If an insurer or another organization controls the record, we will explain how to direct the request without representing that we can change its records ourselves.
